Please fill out each of the three forms below to the best of your availability, prior to your first appointment. 

Payment:

Payment is accepted in the form of cash, check or major credit card.  Checks issued with insufficient funds will incur an additional $35 fee.

Insurance:

We can verify insurance benefits before your initial appointment so that you understand your financial responsibility.

We are in-network with the following companies: 

  • LifeWise
  • Blue Cross Blue Shield (Regence)
  • Health Net
  • Pacific Source
  • Providence insurance  
  • Aetna

Additionally, we can bill out to most insurance companies as long as you have out-of-network benefits.

Cancellation Policy:

All cancelations must be made at least 24 hours in advance. Failure to do so will result in a cancellation fee of $60.
